Look up more info (Coleman Propane can warning)...posted by MadMax.
There are issues with some green cans that the top seal (white) on the can is pushed too far down and crushing the oring below it. This causes the o-ring to pinch/grab the insert of the adapater too hard, holding it down when you remove the mag when filling.
There are some mechanical fixes. MadMax won't say how (liability reasons)...but Colin_S' blog (Google Colin Sun Blog) describes it with some pics I think (or at least used to...this info is about a year+ old).
Anyways...that's my guess as to what's happening.
