The jam is mostly due to the part of the slide that arms the hammer, the little bump that pushes the arm with a little wheel on it, when aiming the gun, situated on the left side. the hammer arm has a spring that pushes it forward and up when armed, and that gets in the way of the slide's bump when the slides' coming in.
