Like everyone else has said, standard name tapes can be bought from 1800nametapes, personally I just added the velcro myself with a sewing machine, but others have their own ways.
As for where which tapes or patches go where, for airsoft, there are no hard and fast rules, just put them where you want. Here is the hard part, use good sense in how much and where you put them, in essence, I can't define that for you, but one will know whether it is right or wong when one sees it.
