I messed with it again and here is what I came up with :
When I rack the slide back, the nozzle gets stuck against the metal chamber but when I shot and it gets stuck it's different. It seems like it's the metal chamber that doesn't move out of the way fast enough so the chamber gets stuck in the slide. When it's stuck like that the nozzle isn't even touching the inside of the metal chamber.
